The Care Management team consists of nursing professionals and licensed social workers. They work to assure that the hospital stay and transition to the community or other care facilities meet appropriate needs. Care Management provides support to patients and their families in the areas of lifestyle transition, planning for future therapy and care for after the patient’s stay. In addition, Care Management assists with social needs during the patient’s stay, develop a comprehensive discharge plan and make referrals to appropriate community agencies for the patient’s return to the community.
